Several of the NFL’s brightest stars are facing major health questions as Week 4 closes, including the San Francisco 49ers’ Brock Purdy, Baltimore Ravens’ Lamar Jackson, and Miami Dolphins’ Tyreek Hill are all dealing with injuries that could alter their seasons. And, in Hill’s case, probably his career.

NFL insider Tom Pelissero didn’t sugarcoat the updates on Tuesday morning. “Tyreek Hill is set to undergo major reconstruction surgery today for multiple torn ligaments,” Pelissero said.

“It is a dislocated knee. It is a torn ACL and more for Tyreek Hill who somehow was smiling on the cart as he left the field. Mike McDaniel said he was cracking jokes with teammates showing leadership in that moment. Suffice to say Tyreek Hill’s 2025 season is over and there are certainly questions about whether or not he will be ready for the start of 2026 and whether he will be on the Dolphins at that point.”

That gruesome scene unfolded on Monday Night Football. And shortly after, NFL Network’s Ian Rapoport confirmed the details. Hill tore multiple ligaments in his dislocated knee, including his ACL, and will undergo surgery that may require multiple follow-ups. At 31, the receiver’s speed-driven game was already approaching decline. And now the timeline is daunting. If his recovery stretches into 2026, Hill could be 32 or even 33 by the time he’s back on the field.

Tyreek Hill’s $11 million injury guarantee and $5 million roster bonus kick in if he’s still on the team in March. But none of his 2026 salary is currently locked in. Cutting him could free up as much as $36–39 million in cap space. A reunion with Kansas City might even be on the table someday, but only if Hill proves healthy and affordable.

Pelissero added, “Tyreek Hill, now 32 years old facing a long road to recovery, much more complicated than just a torn ACL. And as for the Dolphins, coming off of a really important win for that organization last night, they must now move ahead without their top wide receiver.”

However, the Dolphins aren’t the only ones sweating out bad news.

Lamar Jackson’s hamstring leaves Ravens on edge

Baltimore is left in limbo after Lamar Jackson exited Sunday’s loss to Kansas City with a hamstring injury. “Ravens quarterback Lamar Jackson’s status is up in the air for this week’s game against the Texans because of the hamstring injury that forced Jackson to leave Sunday’s loss to the Chiefs in the third quarter,” Pelissero explained.

“John Harbaugh did not add a lot of details … other than to say there was no way that Lamar could have come back into the game even if the score were more competitive. That is certainly an indication that Lamar Jackson is in danger of missing time because of this injury.”

Jackson’s own coach echoed the concern. “No, there was no way he was going back in the game,” John Harbaugh said after the loss. “I know Lamar. If he could’ve gone in the game, he would’ve.”

The Ravens are in a bind. Sitting at 1-3, they play the Texans and Rams before a Week 7 bye. Pelissero suggested Baltimore could consider holding Jackson out through the break, giving him almost a full month to recover. Behind him, options are thin: Cooper Rush, who finished Sunday’s game, and Tyler Huntley on the practice squad. Neither can replicate Jackson’s impact, but they may have to bridge the gap.

Brock Purdy’s toe flare-up clouds 49ers’ Week 5

Meanwhile, San Francisco is dealing with its own quarterback drama. Brock Purdy, who returned in Week 4 after missing two games with shoulder and toe injuries, didn’t come out of the Jaguars game clean. “49ers quarterback Brock Purdy’s status is once again up in the air as well,” Pelissero said.

“Coach Kyle Shanahan told reporters that Purdy complained of real soreness in the toe on Monday after he had missed the previous two games because of that injury. Shanahan said he didn’t indicate it to me during the game but talked to him on the phone last night and said it was starting to bother him.”

Purdy has shown flashes. Completing 48 of 73 passes for 586 yards, 4 touchdowns, and 4 interceptions, but the turf toe remains an obstacle. NFL insider Ian Rapoport noted it was initially a 2–5 week recovery timeline. And Purdy rushed back on the early side. The soreness now raises fresh questions about whether he should’ve waited longer.

“He was listed as did not participate on the estimated injury report on Monday. If Purdy can’t go on a short week Thursday night against the Rams, it will be Mac Jones … getting the ball once again against LA,” Pelissero added.

If he sits, Mac Jones, who went 2-0 as a starter in Purdy’s absence, will get another chance. To guide the 49ers in a pivotal NFC West clash against the Rams.

From Hill’s career-threatening knee injury to Jackson’s nagging hamstring and Purdy’s lingering toe, Week 4 underscored how fragile an NFL season can be. For Miami, Baltimore, and San Francisco, the road ahead could look drastically different. It all depends on how their stars heal—and how quickly their teams adapt.
