Sunday morning was supposed to be a day of running, cheering, and personal goals for nearly 8,000 people taking part in the Milwaukee Lakefront Marathon weekend. Runners from 46 states and 12 countries came together to take on the city’s roads and lakefront course, with many hoping to reach the finish line simply. But during the 13.1-mile half marathon, a runner suffered a medical emergency on the course and was rushed to a local hospital, where the participant was later pronounced dead.

The half marathon began at 7 a.m. on Sunday, October 4, as part of the 44th edition of the Milwaukee Lakefront Marathon weekend. The route takes runners across the Hoan Bridge, one of the most recognizable parts of the course. During the race, a runner suffered a medical emergency along the section of the course near the Hoan Bridge.
WISN reported that shortly after 10 a.m., its weather camera captured first responders loading a person onto a stretcher and into an ambulance near the north end of the Hoan Bridge. The runner was then taken to a local hospital. A runner who spoke with WISN said they saw someone receiving medical treatment near the course. However, officials have not publicly confirmed the exact treatment provided at the scene.
One person who said they witnessed the incident later shared an account on Reddit, writing, “I ran past him on the hoan when they were doing chest compression then I saw them take the defibrillators out. He was so close to the finish line too.”
Soon after that, the participant was taken to a local hospital, where the Milwaukee County Medical Examiner’s Office was later called after the runner was pronounced dead on October 4. Officials have not released the runner’s identity, age or further details about the circumstances surrounding the medical emergency.
Following the death, Milwaukee Lakefront Marathon organizers released a statement expressing condolences to the runner’s family and loved ones. “The Milwaukee Lakefront Marathon extends its sincere condolences to the family and loved ones of the half marathon participant who passed away following Sunday’s event. We are deeply saddened by this loss and are grateful to the medical personnel, first responders, volunteers and public safety partners who support the safety and well-being of participants throughout race weekend.”
The event also has medical and emergency-response protocols in place, such as on-course support from medical staff, protocols for communications and planning with medical and public-safety partners. Organisers said that participant safety is always a key concern in planning and execution of the event.
So far, it is unclear why the runner suffered a medical emergency. However, the Milwaukee Lakefront Half Marathon was not the only half marathon in 2026 to end with the death of a runner following a medical emergency.
Before Milwaukee, multiple runners had already died during Half Marathons in 2026
2026 has already seen multiple runners die during or after half marathons in different parts of the world.
One of the earliest and most devastating cases came at the Leiden Half Marathon in the Netherlands on May 10, 2026. A 15-year-old girl from Oegstgeest became unwell and collapsed while competing in the 21.1-kilometer race. She was reportedly about 4 kilometers from the finish, leaving her close to completing the course. Emergency responders immediately attended to the teenager, with a trauma helicopter also called to the scene, but she could not be saved.
Then, just weeks before the Milwaukee tragedy, the Great North Run in England on September 13 was struck by another devastating incident. The 45th edition of the Newcastle-to-South Shields race attracted around 63,000 runners, but two participants would not make it to the finish.
Two 25-year-old men from northeast England collapsed while taking part in the half marathon and later died. Organizers said the two runners died despite the “very best efforts” of the medical teams who responded to the emergencies.
With another runner now dying following a medical emergency at Milwaukee’s Lakefront Half Marathon, these incidents add to a difficult year for the half-marathon community.
